Newly FDA Approved Treatments for the

Management of Periodontal Disease

The methods historically used in controlling periodontal disease have centered around the control of bacterial plaque - brushing, flossing, scaling, root planing and surgery. A short review of conventional therapeutic strategies for the treatment and long-term management of periodontal disease will be followed by a discussion of newly developed treatment modalities. In this age of minimally invasive soft tissue management, the clinician will be brought up to date on adjunctive therapeutics to optimize clinical results. Recently approved locally delivered therapeutics will be discussed. They is appropriate for many patient types: Maintenance therapy patients, patients who have periodontal pockets that are difficult to instrument, eg, furcations and odd root morphologies and non-responders to scaling and root planning (SRP) with pocket depths > 5 mm who still bleed on probing.

The PerioChip 2.5 mg (chlorhexidine gluconate) is a small chip that is inserted directly into the periodontal pockets with the probing depth > 5 mm. It provides a controlled release of the antimicrobial chlorhexidine directly at the site, exposing the pocket to chlorhexidine for at least 7 days. It was tested as an adjunct to scaling and root planing to reduce probing pocket depth in patients with adult periodontitis. An overview of the clinical studies supporting the safety, efficacy and clinical use of the PerioChip will be presented. The latest research findings, as well as implications for use in practice, will be discussed.

The Atridox product (doxycycline hyclate) 10 % is a subgingival controlled-release product composed of a two syringe mixing system. Upon contact with the crevicular fluid, the liquid product solidifies and then allows for controlled release of drug for a period of 7 days. Atridox is indicated for use in the treatment of chronic adult periodontitis for a gain in clinical attachment, reduction in probing depth and reduction in bleeding on probing.
