Interdisciplinary Management Of Anterior Esthetic Dilemmas: When, Why, And How?

Doctor Kokich is a professor in the department of Orthodontics at the University of Washington in Seattle. He also maintains a private Orthodontic practice in Tacoma, Washington. He has published 16 book chapters, 64 scientific articles, and 48 review articles. He has given over 550 presentations throughout North & South America, Europe, Africa, and Asia. He was awarded the Sicher Research award by the American Association of Orthodontists, and has received the Strang Award (1994), Schluger Award (2000), ICO Award(2001), and the Dewel Award (2002). Dr. Kokich has been elected to Fellowship in both the American College of Dentists and the Royal College of Surgeons of England. Dr. Kokich is Editor of Case Reports for the American Journal of Orthodontics and Dentofacial Orthopedics, Associate Editor of The Angle Orthodontist and Practical Reviews in Orthodontics, and serves on the advisory boards of the Journal of Esthetic Dentistry, Seminars of Orthodontics, Clinical Orthodontics and Research, Dental Traumatology, The Australian Orthodontic Journal, and the British Journal of Orthodontics. He has served as President of both the American Academy of Esthetic Dentistry and the American Board of Orthodontics.
The program will include the following complex treatment situations. Many clinical examples will be used to illustrate the various treatment possibilities for these situations:
1) Midline Discrepancies
• Threshold level or correction of midline deviation
• Keys to solving midline problems
• Relationships of axial inclination and midline discrepancies
• Method for assessing midline deviation
• Restoration of mediolateral inclination problems
• 4 criteria to differentiate the appropriate solution
2) “Gummy Smile”
• Method for evaluating gingiva to lip relationship
• Threshold level for correction of gingiva to lip relationship problems
• 3 treatment options (jaw surgery, periodontal surgery, intrusion)
• 5 criteria to determine when each solution is appropriate
3) “Black Triangles”
• Threshold level for correction of open gingival embrasures
• 3 causes: tooth shape, tooth position, periodontal defects
• Learn how "black spaces" can be corrected
4) Unequal Crown Length
• Threshold level for correction of crown length problems
• 3 possible solutions: periodontal surgery, orthodontic intrusion and restoration, or orthodontic extrusion and equilibration
• Learn the 4 criteria to determine the correct treatment choice
5) Single tooth maxillary lateral and mandibular second premolar implants
• How much space is neccesary for implant and crown
• When to place implants
• How to develop the ridge with tooth movement
• Effect of timing on bone thickness for implant placement

How to Reduce High Bites and Open/Tight Contacts

               Too many models look perfect on the model only to fall drastically short of expectations at the seating appointment.  The necessary time-consuming adjustments to the occlusion and/or contacts can leave you and your patient feeling frustrated.

               Finding a laboratory that can deliver a completed case requiring little to no adjustment is the biggest challenge a dentist faces when prescribing a crown or bridge.  To identify factors that determine whether a crown will fit, a study was recently undertaken to explore dentist-laboratory interaction and how it ultimately affects case outcome.

               In “How to Reduce High Bites & Open/Tight Contacts,” Dr. Robert Cowie will review the study results to shed light in what goes wrong and to offer suggestions that will prevent future mistakes.

Seminar highlights include:

·         Review of the study, analyzing occlusion and contacts of 30 single-unit crowns

                                               Dr. Robert Cowie is a 1976 graduate of Northwestern University Dental School and practices in Jacksonville, Florida.  He holds membership in numerous professional organizations including the Jacksonville Dental Society [past President], the AGD, the American Equilibration Society, the Florida Academy of Dental Practice Administration, and the Pierre Fauchard Academy. He completed the continuums at the Center for Advanced Dental Studies and the L.D.Pankey Institute, Dr. Cowie has lectured nationally while continuing to practice a minimum of 30 hrs. a week chairside.
